When I was in Surat Thani I bought a bus and ferry combo ticket from Seatran. It was 250Baht and it included a ride on the express bus from Surat Thani bus station to Don Sak pier and then a 2 hour trip by ferry to Koh Samui, arriving at Na Thon pier.
